Age 3 - Age 5

We provide a variety of support for adolescents.

When a child turns 3, the team will transition to coordinating services to empower the child to succeed. The Individual Support Coordinator (ISC) will help identify and connect with services available to the child in the community and through the local school districts.

Supporting Children in Their Development

Fairfield DD connects children and families with community resources to help them achieve greater independence and meet their goals. We believe in an inclusive community that values a child’s gifts and talents. Our team works closely with each child and family to understand their skills, relationships, and goals, creating a collaborative plan tailored to their needs as they start their education journey in school.

Each child is assigned an Individual Support Coordinator (ISC) to guide them and their family, celebrate successes, and adjust plans as needed. Our specialized staff, including behavior support and resource management, ensure that families and children have access to necessary funding and services as they develop skills and strengths at an early age, leading to a more independent and fulfilling life.
