Provider Resources

Providers are an important part of our community, working with people to help them reach their goals. The Ohio Department of Developmental Disabilities certifies providers in Ohio. Below are resources to help you be successful!

Provider Certification

DODD licenses providers of services to people with developmental disabilities. There are 2 types:

Becoming a Provider?

Applicants must meet qualifications, pay a non-refundable fee, and include supporting documentation with their application. Use the resource list below to ensure you complete your application.

Provider Compliance

Ensuring compliance with state and federal rules is one way DODD supports the health and welfare of people with developmental disabilities who access services across Ohio.

Provider Guides

Annual Review

Annually, providers are required to review their unusual incidents for trends and patterns.

Quarterly Review

All agency and independent providers shall maintain a log of all unusual incidents. Submit for review quarterly to Fairfield DD by the dates below.

1st Quarter: Jan- March
Due April 15th

2nd Quarter: April-June
Due July 15th

3rd Quarter: July-September
Due October 15th

4th Quarter: Oct.-December
Due January 15th 

MUI Reporting

A Major Unusual Incident (MUI) means the alleged, suspected, or actual occurrence of an incident when there is reason to believe that the health or welfare of an individual may be adversely affected, or an individual may be placed at a likely risk of harm, if such individual is receiving services through the developmental disabilities service delivery system or will be receiving such services as a result of the incident. There are three categories of major unusual incidents that correspond to three administrative investigation procedures.

If you are concerned that a MUI has occurred, contact the MUI department at 740.652.7220 Monday - Friday, 8 am to 4:30 pm. If you are reporting outside normal business hours, call 211 or 740.687.8255. If you are sending in a report after speaking to a MUI staff or designated MUI staff member, send to report@fairfielddd.com by next business day at 3:00 p.m.

Health and Safety Toolkit
The Ohio Department of Developmental Disabilities has a Health and Safety Toolkit on its website. The Toolkit includes information specifically grouped for families, providers, and county boards of developmental disabilities, and features helpful forms and fact sheets.
