Introduction and Eligibility

Those interested in receiving services from Fairfield DD first need to complete the application. Eligibility requirements differ depending on the applicant’s age. Referring a child age 0-3? Click here.

Step 1: Gather Materials
1. Copy of Social Security Card
2. Copy of Birth Certificate
3. Copy of Insurance Card

Step 2: Complete application. Submit documents and application to eligibility@fairfielddd.com

Step 3: Speak with Introduction and Eligibility Specialist to plan next steps.
Step 4: Submit qualifying documentation.

 

Eligibility Requirements by Age

For children ages 3-5, there must be at least two areas of developmental delay and a qualifying diagnosis as documented by a qualified professional.

For children and adults ages 6+, there must be a diagnosed developmental disability from a qualified professional and must also be eligible according to the Children’s/ Ohio Eligibility Determination Instrument (C/OEDI).

For those already receiving services, eligibility is re-determined at ages 3, 6, and 16.

Frequently Asked Questions

  • Referrals should come from the applicant or their legal guardian.

  • The intake process differs depending on applicant’s age and specifics which will be discussed with you during the initial request for services. For any age, the intake department will review documented diagnoses. For ages 6 and up, completion of the Children’s/Ohio Eligibility Determination Instrument will also be required.

  • The length of the process depends on each case. If the intake department is requesting records from outside agencies, it can take 30 days to process record requests. Individuals or guardians can provide records which can expedite the process. Examples of records may include: Psychological Evaluations, Medical evaluations, hospital records, Evaluation Team Reports (ETR), or Individualized Education Plans (IEP). Once supporting documentation is received, the C/OEDI will be scheduled.

  • Fairfield DD provides case management through Individual Support Coordinators (ISC’s) to individuals that qualify for services. ISC’s help connect you to community resources and services that may meet those needs. ISC’s coordinate a host of services including social, medical, education, and employment. Learn more

    Fairfield DD also provides a program called Family Support Services (FSS). It is a taxable income based program that provides financial assistance to individuals who live with their family member. Learn More
