Your Partner in Accessing the Support You Deserve
You have strengths, skills, supports, relationships, and resources which make you unique. Your Individual Support Coordinator (ISC) meets to learn about all of these in your life. Through this discussion, the ISC will work to determine any gaps in support. Each one of us come with different needs and Fairfield DD staff work to learn the specifics of your needs and situation.
Our staff are trained to connect you with resources to fill any identified needs. Some of these may be familiar such as family and friends, community resources, or insurance. Others may be new such as Medicare, Medicaid, or Medicaid Waiver Services. Your ISC will help connect you with what fills your need and help you navigate because option has each has its own set of requirements.

Family Support Services:
Families are eligible to apply for Family Support Services if the person lives in the home for funding.
