As they transition into and through adulthood, the Individual Support Coordinators at Fairfield DD work to connect people with resources within the community to help them reach their goals and make progress toward independence and a fulfilling life.

Fairfield DD connects people with community resources to help them achieve greater independence and a fulfilling life. We believe in an inclusive community that values everyone’s gifts and talents. Our team works closely with each person to understand their skills, relationships, and goals, creating a collaborative plan tailored to their needs.

Each individual is assigned an Individual Support Coordinator (ISC) to guide them, celebrate successes, and adjust plans as needed. Our specialized staff, including behavior support and resource management, ensure that individuals have access to necessary services and supports to live life on their terms, from job opportunities to personal support systems.

Residential Services

Let our dedicated Individual Support Coordinators help you navigate the housing choices and options available to you! This may include living alone or with family, friends, or a significant other. It may include living with one or several roommates. We will help you navigate resources and funding as you find what is right for you.

Intermediate Care Facilities

Another option is an Intermediate Care Facility (ICF), which is a place where a person with a disability can live and receive services in a group setting.
