Eligibility requirements for every age group.

As you explore Fairfield DD services, here's what to expect.

We’re here to recognize your value to the community and get to know your interests, strengths, and future goals. We connect you with local resources and guide you through support options that fit your vision. To qualify, a developmental disability must appear before age 22, last throughout life, and meet eligibility criteria based on functional limitations.

To begin, Select an Age group:

Birth - Age 2

Age 3 - Age 5

Age 6 - Age 15

Age 16 - Adult

Birth - Age 2

Fairfield DD partners with Help Me Grow and Lancaster-Fairfield Community Action to provide Early Intervention Services.

Anyone can make a referral by calling the intake specialist at 740-681-4881, calling 1-800-755-GROW (4769) or submitting the online referral.

What happens next?

The intake specialist will contact you at a time that fits your schedule to talk about the child’s development and complete a screening to determine eligibility. Eligibility is based on at least one of the following: Diagnosis, Delay found using a whole child development evaluation tool, or Informed Clinical Opinion.

Age 3 - Age 5

You will need to gather these necessary items:

1
Make a copy of the child’s records, including:
child’s social security card
child/family’s insurance card
child’s birth certificate
and at least 1 of the following:
Records of assessment through the school district, including IEP (individual education plan) or the ETR (Evaluation Team Report)
Psychological evaluation completed by a psychologist

Fill out the form and submit the child’s records.

What happens next?

Once all records are received, you will be contacted by the Introduction and Eligibility Specialist to discuss next steps.

Age 6 - Age 15

You will need to gather these necessary items:

Make a copy of the child’s records, including:
child’s social security card
child/family’s insurance card
child’s birth certificate
Records of assessment through the school district, including IEP (individual education plan) or the ETR (Evaluation Team Report)
Documentation of a qualified diagnosis from a qualified professional (this is most commonly in the form of a psychological evaluation but could also be through medical documentation)

What happens next?

Fairfield DD will contact you to schedule an assessment called the COEDI. This assessment typically takes an hour and a half to complete. It is important for the child to be a part of the interview.

Age 16 - Adult

You will need to gather these necessary items:

Make a copy of the person's records, including:
social security card
insurance card
birth certificate
Documentation of a qualified diagnosis from a qualified professional (this is most commonly in the form of a psychological evaluation but could also be through medical documentation)

Fill out the form and submit the person's records.

What happens next?

Fairfield DD will contact you to schedule an assessment called an OEDI. This assessment typically takes an hour and a half to complete. It is important for the person requesting to services to be a part of the interview.
