Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

Individual is a sweet young man who likes to make others smile. Individual is working on making more independent choices. His provider would need to be attentive as he is always on the go and is very active. Assist him to use his communication device to communicate his needs, wants, and other preferences. He will require someone who is able to stay active with him and at home and in the community and who is able to take him to a park, swimming, or find other ways for him to stay active are a plus especially on trash day. Individual likes to do things that make him happy like basketball, play with bubbles, and listen to music. He will need positive interactions. Other activities he enjoys including playing outside, doing crafts, playing on his iPad. He needs choices of activities so he can decide what he wants to do. Provider will need to be able to take him to his ADS program each morning and pick him up afterwards. During this time he requires close supervision, consistent interactions, and routine. He has a positive behavior plan so training will be provided, documented, and teamwork is key. Individual is large in statue and can be intimidating but is really gentle once he knows you. Provider must know how to appropriately handle behavioral disruptions that may include him becoming destructive. He thrives on routine and playful interactions. He will have a behavioral support coordinator involved initially to help assist with learning techniques that work best for him and his family.
Best ways to communicate with the individual:
Individual wants to be communicated with verbally. He has a communication device to assist him with verbalizing his needs and wants.

He has a wheelchair for longer distances or for when his legs are bothering him. Household chores: He participates with support washing dishes, putting away laundry, and cooking. He has positive behavior supports in his service plan to assist him with personal growth and with furthering independence. Provider must be comfortable with dogs.

Say his name so he knows you are talking with him and speak clearly. He uses words and body language/behavior to communicate. Give him extra time to process information to formulate a response to questions or when needing to make a decision. He often interacts with people by sharing music with them, and will sing part of a song and wait for someone else to finish the song. He will sing songs based on what mood he is in.

Person is a very kind and helpful man. He would like to have a provider to assist him with gaining access to his community, while and also providing assistance with him participating in events in the deaf community that he chooses, which is most important to him.
Best ways to communicate with the individual:
This gentleman is deaf. A provider fluent in sign or at least has some working knowledge of sign language is preferred, but not required. Most communication is done via text or phone with his mother, who will always have his most current work and personal schedule. This gentleman is able to communicate via writing things down and reading what others have written. He is able to get his point across with showing you or taking you to what he is wanting you to know or see.
