What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

Searching for Medicare/Medicaid Certified Agency (Type 60) or Medicaid Independent Nurse(s) for 10-12 hour Private Duty Nursing shifts for individual diagnosed with Allergies, Acid Reflux, Chronic Autonomic Storming, Central Sleep Apnea, Chronic Respiratory Failure, Constipation, Trach and Vent Dependent, DO of Urinary system, Familial Dysautonomia (Riley-Day), ID, Microcephalia, Quadriplegia NOS, Spastic Quadriplegia Cerebral Palsy, Visual Impairment, Adrenal Insufficiency. Parents assist with medical care and mobility tasks when needed and will train nurse(s) on daughter's care. Nurse will assist individual to attend a day program one day a week (Thursdays) once they are comfortable with her care. Until then, the parent will assist their daughter to attend the day program. The goal is to keep the patient well and avoid hospitalizations and to increase socialization/community engagement. Nurse may be asked to assist the parent to take daughter to medical appointments.

The patient is unable to speak. She likes to be talked with as the adult she is and she enjoys hearing her nurses chat with her about their day and experiences etc..... She communicates through facial expressions and body language and is learning to use her new Tobii Eye Gaze device for increased communication.

Individual can communicate verbally but she does have a communication device because she is not always fully understood by those who are not always around her. The family is working with the behavioral support program to help establish techniques that work for the individual and her family to minimize the conflict when she is frustrated.

It is important to note that he is unsure at the moment what he desires to work on specifically, but his parents want him to have this opportunity to explore ways for him to become more independent.
Best ways to communicate with the individual:
It is best to communicate with him directly and clearly, and give him the space and time to grow comfortable. He has a history of shutting down or expressing his frustration verbally when asked to do something he does not want to do. At these times, it is best to give him space and come back to the activity at a later time.

Best ways to communicate with the individual:
She likes to be included in activities and conversations and spoken to in a calm voice. Staff should avoid saying "no" or "stop it" and instead redirect.

Say his name so he knows you are talking with him and speak clearly. He uses words and body language/behavior to communicate. Give him extra time to process information to formulate a response to questions or when needing to make a decision. He often interacts with people by sharing music with them, and will sing part of a song and wait for someone else to finish the song. He will sing songs based on what mood he is in.
