Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

He lives in Thornville (Buckeye Lake area). He and his parents would like him to work on communication and decision making without prompts or reminders. He requires full support with all personal care and hygiene tasks, as he is unable to perform these independently. He also needs assistance with daily living skills, including feeding, dressing, and mobility. For communication, he primarily uses a PRC LAMP device and benefits when staff model its use during conversations. He requires guidance and supervision for safety, including navigating environments, responding to potential risks, and remaining safe at home and in the community

He is sweet young man who when you are around, you can't help but smile. He has a guardian but is working on making more independent choices. One will need to be attentive as he is always on the go. He can also elope and become hyper-focused on trash bins. Assist him to use his communication app to communicate his wants, needs, and preferences. He will require someone who is able to stay active with him at home and in the community, and who is able to take him to a park, swimming, or find other ways for him to stay active are a plus especially on trash day. He likes to do things that make him happy like play basketball, play with bubbles, and listen to music. Will need positive interactions. Other activities he enjoys - playing outside, doing crafts, playing with his iPad. He needs choices of activities, so he makes his own decisions to do what he wants. One will meet at the ADS bus and escort him home and provide the proper supervision. He has a positive behavior support so training will be provided, documentation and teamwork is key.
