If you are looking for opportunities to provide services, be sure to review all RFP’s and respond to any that you meet all listed requirements. Please fill out the form for each posting of interest. Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

He likes a variety in the activities he is doing. Work on his Outcomes. 1. Is physically active to preserve and improve mobility. 2. Is selling items he has in storage so he can earn money to buy new items he wants but does not have room to store.

He likes others to be nice and respectful when talking to him. He enjoys talking and joking with people when engaging in conversations.

To have support in helping him find ways to be active. Needs help with laundry, cooking meals, and cleaning his home. Help him explore different kinds of food. Individual can make simple meals but will need total support in cooking more complex meals. Individual does require assistance using the stove he needs supervision and support helping him cook meals on the stove.
Best ways to communicate with the individual:
Individual prefers someone who communicates clearly and concisely. It is important to ensure that every party is on the same page. Speak directly to individual, do not speak too fast so he will understand what you are saying, give him time to process and respond to what you asked.
