Financial Help for Families Providing In-Home Support.

Fairfield DD’s Family Support Services (FSS) offers monetary assistance for individuals and families navigating daily life when other resources are not available. Reimbursable services and supports are tailored to the unique needs of the person, regardless of age, living in the family home.

Getting Started

We are here to help you navigate the Family Support Services (FSS) application process to support individuals who qualify for Fairfield DD services. New to the eligibility process? Use our Eligibility Tool to learn about the requirements based on the age of the person.

Process Overview:

  1. Complete the Taxable Income Verification form and submit it to familysupport@fairfielddd.com
  2. The FSS Coordinator will determine eligibility and  funding level and you will receive notification and next steps via email.
  3. Plan items you hope to purchase to support the person and submit the Request for Approval to the FSS Coordinator before making the purchase. Without this, we can’t ensure reimbursement.
  4. You will be notified of approval or denial of payment. Once approved, you can purchase the item and submit the receipt to the FSS Coordinator.

Applicable Services

These are just a few ways FSS funds might be used based on the needs of the person:
Therapy Services
Medical Supplies such as catheter supplies, stoma seals/rings, and medical pads that are not otherwise covered.
Incontinence Supplies Disposable: diapers, wipes, pads, plastic sheets, etc. for ages 3 and up that are not covered.
Special Dietary Items like specialized formula, shakes, or thickeners
Enrichment Memberships such as COSI, Zoo and Swim
Special Equipment such as modifications, sensory items, therapy balls, communication devices, adaptive car seats, and strollers
Respite Care

Keep in mind:

The request for assistance must relate directly to an individual’s disability and often requires a professional recommendation. Family Support Services is administered through Fairfield DD with local funding and are processed through the Fairfield County Auditor’s Office. All processes must meet auditor guidelines and must be approved before being purchased. All family allocations are based on the availability of funds.

Summer Scholarship

Summer scholarship is a program that provides some funding support for summer enrichment activities for school-age students. Designed to continue or extend learning, the funding can be tailored to the student’s interest. Funding is based on the age of the student and school enrollment status.

Application Process

  1. Beginning March 1, families can begin registering their student for the program. All registrations should be received by May 31.
  2. Once registered, allotment letters and request forms will be sent to your mailing address. Please remember, this is a first come, first served basis.
  3. Submit your request form to the Family Support Services team at familysupport@fairfielddd.com.
