Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

Person is very social, easy-going and embraces life. He is requesting assistance needed to complete his daily hygiene. Completion of health related tasks of assistance with vent GTube, administration of fluids, nutrition, medications through G/J Tube. Supports should be familiar with seizure protocols and maintain the appropriate level of family delegation and or necessary certifications. He is never left alone as it is important to monitor his level of comfort, engage him in conversation, and adapt activities of interest to his abilities and involvement.
Best ways to communicate with the individual:
Individual communicates primarily through facial expressions and reaching for things. Individual is able to say some words, but mostly makes vocalizations. When he gets loud, he is usually excited and thinks something is funny. Individual likes to be spoken to in a normal tone of voice, he is able to understand you. Supports should provide him with choices whenever possible. Supports should provide two options and he will choose by reaching for what he wants or by looking at the item.

Individual enjoys Legos, arts and crafts, and animals (especially horses). He is described as caring, helpful, fun, engaging, determined, curious, strong-willed, and adventurous. A strong, calm male role model is important so male staff is preferred.
Best ways to communicate with the individual:
Just talk to him, he likes to be included in conversations. He has attempted to use a communication device (Tobii Dynavox and LAMP ) in the past but is increasing his comfort and confidence in communicating verbally. All scheduling and communication will be handled by parent via phone, in person, or text.

Best ways to communicate with the individual:
Individual uses an eye gaze communication device to make requests and answer questions. He also communicates his thoughts and feelings through vocalizations, laughing, crying and body language. He also answers yes or no questions by tapping on someone hand. He uses picture cards as well.

Individual communicates by using some spoken words and by leading others to what he wants. Individual smiles, hums, and laughs to indicate when he likes something. If he needs help he will say help please. Individual needs others to use a calm voice with him, and give simple one step instructions. He does best when he has time to process things so he doesn't feel overwhelmed.

This individual also needs assistance getting to and from medical appointments.
