Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

Individual needs assistance with some personal hygiene, cooking and meal preparation, some cleaning in the apartment. Individual has a small dog and lives alone. Enjoys going to the dog park, amusement parks, concerts, spending time outdoors, listening to music, etc. The individual would like to work on navigating her community on her own and making her apartment functional. She needs staff who are prompt, reliable, and in constant communication with changes. Potential for out of town/out of state trips.
Best ways to communicate with the individual:
Talk to her in person, on the phone, text, email, etc. To speak slowly and calmly are helpful when communicating with her.

Expected progress towards this outcome includes attending social events, meeting new people, becoming an integral part of the community organization/event/club, and accessing transportation options to/from events. Encourage and assist this person to work on developing additional social skills to help this person interact positively with his peers and others in the community.
Best ways to communicate with the individual:
To set up initial schedule, communicate any change in schedule, and arrange pick up/drop times via phone call with parent/guardian. When talking with this person provide time to process and time to respond to questions and while in conversation; repeat or rephrase the question if needed and provide additional time to respond.

Encourage use of communication device/eye gaze. Assist in transporting her to places outside of the home. Keep in sight supervision while outside of the home at all times. Take places she wants or likes to go. Assist in all aspects of meal preparation including eating and clean up. Assist with all routine g-tube feedings and cleaning/care of G Tube equipment.
Best ways to communicate with the individual:
Individual communicates with devices, modified sign language or by pointing, vocalizing, etc. Staff can provide choices or use process of elimination to determine what individual wants or needs.
